I don't get it.  Why is my doctor billing me, and my insurance company?  So I went to the doctor like 3 months ago, and now I get a bill to pay them for my doctor's visit.  The only problem is that at the time I presented my medical insurance card and paid a $25 dollar copay and that was that.  Then I get a statement from my insurance company that they had paid something and then I get a bill from the doctor for the full amount.  Are they trying to get money from me and my insurance company?  I'm totally confused.  I don't know who I should pay.  I'm certain if I pay the doctor I'll never see that money again, but at the same time, I don't want to be reported for failure to pay.  God, why does health care have to be so friggin complicated!  I don't mind paying, I just don't know what I'm suppose to pay or what.  Grrrr....

Call the doctor's office and ask them what the bill was for, and why it wasn't put through your insurance company.  Welcome to the insurance company nightmare.  (Maybe somebody can drag Kang out of FaceBook to answer your question in more detail.)
